Fibres or grain lifting and pulling away instead of shearing cleanly, almost always where the tool exits the work or crosses the grain. Backing the cut with sacrificial stock, scoring the exit line first, or switching to a climb cut all fix it. Sanding afterwards only spreads the crater.
